To the Editor.— In their CASE REPORT on labetalol hydrochloride therapy for acute aortic dissection, Grubb et al1used a 20-mg bolus followed by a continuous infusion of 2 mg/min for two days. The total intravenous dose was therefore 5.78 g in 48 hours. The manufacturer suggests a maximum dose of 300 mg/d.2Could the authors please comment on the dosing

The effectiveness, safety, and pharmacodynamics of repeated doses of intravenous labetalol for rapid reduction of severe hypertension and of subsequent oral labetalol dosing were studied. Twelve patients with severe hypertension were admitted to the hospital after the withholding of antihypertensive therapy for 2 to 14 days.

Emergency Treatment of Severe Hypertension with Intravenous Labetalol

The effects of intravenously administered labetalol on blood pressure and pulse rate were examined in 17 patients with severe hypertension. Prompt and sustained falls in supine blood pressure and pulse rate occurred in ten patients (responders), but seven patients showed little or no change in either measurement (non-responders).

Labetalol inhibits alpha- and beta-adrenergic receptors. Systemic and regional hemodynamic alterations after intravenous labetalol and its cardiovascular reflexive and metabolic effects were evaluated in 12 subjects with mild to moderately severe essential hypertension.
