To reduce the complication rate associated with laparoscopic surgery and to improve training, several simulators have been incorporated into training curricula and skills courses. We discuss the advantages and disadvantages and compare the different types of simulators available. We also reviewed the literature to assess the acquisition of skills using

Surgical problems in the patient with HIV and AIDS are becoming more commonplace as the incidence of HIV seropositivity increases and patients with AIDS are living longer. Laparoscopic surgery is being applied more frequently in the diagnosis and therapy of these patients' problems as more surgeons become familiar with the techniques.

Diagnostic laparoscopy continues to play an important role in the accurate evaluation of patients with abdominal disorders. Combined with laparoscopic ultrasound, it is highly accurate in the staging of intra-abdominal malignancies, and it is superior to transcutaneous ultrasonography and computed tomography.

Many of the complications of conventional closed, or sharp, laparoscopy result from the use of insufflation needles and sharp trocars. These instruments are not essential elements of the technique as laparoscopy can be easily performed with a small umbilical incision, entering the abdomen under direct vision. This variation is called open laparoscopy.

Today, female sterilization is most easily accomplished by single-puncture laparoscopy and, ideally, is carried out under local anesthesia. In experienced hands, electrocoagulation offers slightly fewer failures if a large portion of tube or two segments are destroyed.

Urologic laparoscopy initially was confined to a diagnostic role or the treatment of benign conditions. Many of these initial procedures, however, have been abandoned because they offered no significant benefit over open surgery. The treatment of urologic malignancies, however, recently has emerged as the most common indication for laparoscopic ...
