2012Lower GI hemorrhage is defined as bleeding that occurs from the bowel distal to the ligament of Treitz. Majority of patients with hematochezia bleed from the large bowel, but in 10-25% of patients, the small bowel is the source of bleeding, and it poses difficult diagnostic dilemma.

ED evaluation of these patients begins with history directed at determining the severity and amount of bleeding, and eliciting symptoms of volume depletion. Physical examination determines orthostasis, exclusion of an upper GI source for bleeding, and rectal examination.
