Anterior Capsular Reconstruction With Dermal Allograft Augmentation for Multidirectional Shoulder Instability

open access: yesArthroscopy Techniques, EarlyView.
Abstract Multidirectional shoulder instability, particularly in patients with underlying connective tissue disorders poses a surgical challenge. After exhaustive nonoperative treatment, surgical intervention is focused on restoration of the anterior and inferior capsular restraints.

IL‐31 levels correlate with pruritus in patients with cholestatic and metabolic liver diseases and is farnesoid X receptor responsive in NASH

open access: yesHepatology, EarlyView., 2022
IL‐31 levels correlate with pruritus in patients with cholestatic and metabolic liver diseases Abstract Background and Aims Pruritus is associated with multiple liver diseases, particularly those with cholestasis, but the mechanism remains incompletely understood.

How to Set Anterior and Posterior Tibial Translation for Combined Anterior Cruciate Ligament and Posterior Cruciate Ligament Reconstruction

open access: yesArthroscopy Techniques, EarlyView.
Abstract Anterior and posterior cruciate ligament (ACL and PCL) reconstruction poses the risk of residual anterior or posterior tibial translation, respectively. As a result, patients may have residual knee instability and laxity, which can contribute to poor outcomes and graft failure.

Janus kinase 2 inhibition by pacritinib as potential therapeutic target for liver fibrosis

open access: yesHepatology, EarlyView., 2022
Diagram of the activation of the profibrotic and procontractile Janus kinase 2 (JAK2)/Ras homolog family member A/Rho‐kinase pathway and the inhibition of phosphorylated JAK2 by pacritinib to inhibit hepatic stellate cell activity. Abstract Background and Aims Janus kinase 2 (JAK2) signaling is increased in human and experimental liver fibrosis with ...

Challenging Resection Comes in Many Forms: Addressing Difficult Cam Morphology

open access: yesArthroscopy Techniques, EarlyView.
Abstract Femoroacetabular impingement with cam morphology that fails nonoperative management can be addressed with hip arthroscopy and cam resection. There exists a wide variation of cam morphology due to various etiologies that can make some cam lesions more challenging to address. A thorough preoperative evaluation and the utilization of a variety of

A shared mucosal gut microbiota signature in primary sclerosing cholangitis before and after liver transplantation

open access: yesHepatology, EarlyView., 2022
In this study of the gut mucosal microbiota in primary sclerosing cholangitis (PSC), we found consistent microbiota features associated with PSC and recurrent PSC, PSC with inflammatory bowel disease and a persistent gut dysbiosis after liver transplantation Abstract Background and Aims Several characteristic features of the fecal microbiota have been ...

Tips and Tricks for Hardware Removal and Allograft Fixation After a Failed Latarjet Procedure

open access: yesArthroscopy Techniques, EarlyView.
Abstract Although relatively rare, recurrent anterior shoulder instability requiring revision surgery after a failed Latarjet procedure can result in complications such as graft malposition, graft osteolysis, broken hardware and/or infection. Revision surgery after a failed Latarjet is a challenge due to a combination of technical factors such as ...

Trans‐Tendon Suture‐Bridge Knotless Proximal Pulley Repair With Bioinductive Patch Augmentation for High‐Grade Partial Hip Abductor Tendon Tears

open access: yesArthroscopy Techniques, EarlyView.
Abstract This technical note describes a knotless trans‐tendon proximal‐pulley repair with distal suture‐bridge fixation and bioinductive collagen patch augmentation for high‐grade partial gluteus medius and minimus tears. The technique preserves intact tendon fibers, restores broad footprint compression, and supports biologic healing through a ...

Oral vancomycin solution is superior to capsule in inducing clinical biomarker and endoscopic remission in children with atypical ulcerative colitis

open access: yesBritish Journal of Clinical Pharmacology, EarlyView.
Aims Atypical colitis (presenting reverse gradient colitis, backwash ileitis or rectal sparing) is associated with primary sclerosing cholangitis–ulcerative colitis (PSC). Oral vancomycin has been used to manage paediatric atypical colitis with/without confirmed PSC. Different preparations had shown different efficacy.

Blood pressure effects of SGLT2 inhibitors and GLP‐1 receptor agonists: Mechanisms, trial evidence and Real‐world data

open access: yesBritish Journal of Clinical Pharmacology, EarlyView.
SGLT2 inhibitors and GLP‐1 receptor agonists modestly lower blood pressure across diverse patient populations, including those without diabetes. These effects appear largely independent of glycaemic control and offer additive value in high‐risk patients with overlapping comorbidities.
