Traumatic occlusion of the middle cerebral artery

✓ A case is reported of a patient rendered unconscious in a motorcycle accident. After a 6-hour lucid interval, he became unconscious again, and a left hemiplegia was noted. Arteriography demonstrated complete occlusion of the right middle cerebral artery. Comparable reported cases and theories of pathogenesis are discussed.

Schizencephaly with occlusion or absence of middle cerebral artery

In a study of 160 infants with prenatal and/or perinatal risk factors for brain damage, we observed three cases of schizencephaly. All cases were unilateral, and the clefts had open lips. In two cases, magnetic resonance angiography showed occlusion or absence of the middle cerebral artery (MCA) on the affected side.

Acute operation in cases of middle cerebral artery occlusion

Abstract The authors describe their personal experience with middle cerebral artery embolectomy performed in four patients within 6 hours after the start of clinical symptoms. The work is of a preliminary nature. No conclusion can be drawn as to the ultimate value of this treatment, and further clinical trials seem justified.

One hour after middle cerebral artery occlusion, the regional blood to brain transfer coefficient of alpha-aminoisobutyric acid was determined in eight barbiturate anaesthetized rats. The transfer coefficient (microliter/min-1/g-1) was significantly higher in the ischaemic cortex (10.6 +/- 2.3) than in the contralateral cortex (6.5 +/- 1.0).

Outcome was studied prospectively in 28 consecutive patients with occlusion of the middle cerebral artery (MCA). They comprise a subgroup of 101 consecutive patients with TIA or stroke less than or equal to 75 years of age, admitted within 72 h after the stroke. Cerebral angiography and CT-scan were performed within 1-2 days of admission.

A 65-year-old woman with chronic atrial fibrillation was admitted for an elective exchange of an implanted defibrillator for idiopathic dilated cardiomyopathy. To facilitate this procedure, warfarin was withheld for 5 days. Before the procedure was performed, acute-onset right hemiparesis and expressive dysphasia developed.

Four patients with arteriographically demonstrated occlusion of the main trunk of the middle cerebral artery were found, by subsequent arteriograms or postmortem examination, to have middle cerebral artery patency. Similar cases have been reported in the literature.

To mimic cerebral ischemia leading to stroke in man, many animal models have been made which occlude brain arteries, transiently or permanently. These models are aimed at studying the morphological changes and the pathophysiology of stroke, and at screening and testing new neuroprotective or thrombolytic drugs.
