Prognosis in monoclonal gammopathy of undetermined significance

Eighty‐seven patients with monoclonal gammopathy of undetermined significance (MGUS) were followed for a period of 1–20 years, median 91 months. Transformation to multiple myeloma occurred in 14 patients of whom seven died as a consequence of the disease. There were 13 unrelated deaths.

[Monoclonal gammopathy of undetermined significance].

Monoclonal gammopathy of undetermined significance (MGUS) is an asymptomatic disorder with serum monoclonal immunoglobulin less than 30 g/l. It preferentially affects elderly patients; MGUS prevalence is about 1% in the general population and about 10% in patients aged over 80 years.

IgD monoclonal gammopathy of undetermined significance.

We describe here a case of monoclonal gammopathy of undetermined significance (MGUS) of IgD type. A trace of serum M-protein was noticed on electrophoresis when a Japanese man visited his primary care physician for medical examination. However, he was asymptomatic. Serum immunoelectrophoresis showed IgD lambda M-protein.

In the majority of monoclonal gammopathies it is not possible to demonstrate an association with an underlying disease related to multiple myeloma, Waldenström's macroglobulinemia, malignant lymphoma or amyloidosis. After a lapse of months to years, however, some patients will develop one of the diseases mentioned.
