症例は79歳女性で,他医で上部消化管内視鏡検査を施行したところ十二指腸潰瘍を指摘されたため同院でプロトンポンプ阻害薬を処方の上経過観察されていた.約8か月後,貧血が指摘され上部消化管内視鏡を再検したところ十二指腸球部潰瘍浸潤型病変が認められ,生検にて腺癌と診断された.腫瘍は幽門輪前部大彎側への浸潤を伴う十二指腸球部下面を主座とする直径約40mmの3型病変であった.十二指腸球部癌の診断で膵頭十二指腸切除術を施行した.病理組織学的に腫瘍はムチンを多く含む粘液癌であり,粘液染色でMUC5AC,MUC6陽性,MUC2陰性の胃型形質を有していた.十二指腸球部癌はまれな疾患であるが,その中でも粘液癌は極めてまれであり報告する.
