We have described a 28-year-old male sheepfarmer who had fever, headache, chills, malaise, and aortic insufficiency. Echocardiography revealed a tricuspid aortic valve with a large vegetation on the right cusp, an enlarged left ventricle, and diastolic flutter of the mitral valve. Repeated blood cultures were negative.

Murine Typhus in Caldas, Colombia

In the north of Caldas, Colombia, febrile syndromes with positive Weil-Felix reactions have been reported as Murine typhus to the national health authorities. We used indirect immunofluorescence assay (IFA) of serial paired samples to confirm the diagnosis of murine typhus in 14 of 120 patients with a compatible febrile syndrome.

Ciprofloxacin Treatment of Murine Typhus

A 31-year-old woman developed fever, severe headache, myalgia and rash 3 weeks after a visit to Cyprus where she experienced insect bites in her bed. Treatment with ciprofloxacin resulted in a remarkable and rapid effect. Endemic typhus was proved by serology.
