AbstractA patient with a chief complaint of epestaxis coughed persistently during her visit to the emergency room. Questioning revealed she also had vaginal bleeding and occasional bloody sputum. Gynecological examination, a Pap smear, and pathological evaluation showed choriocarcinoma. A chest X ray disclosed metastatic pulmonary choriocarcinoma.

Nasal delivery is a potential alternative for systemic availability of drugs restricted to intravenous administration, such as peptide and protein drugs. Although nasal delivery avoids the hepatic first-pass effect, the enzymatic barrier of the nasal mucosa creates a pseudo-first-pass effect.
