Transurethral resection for ejaculatory duct stenosis and oligospermia

Four patients with oligospermia were treated for ejaculatory obstruction after diagnosis by semen analysis and vasography. Ejaculatory obstruction was caused by urethral catheterization or infection in all cases. Transurethral resection of the posterior prostatic urethra produced fertile levels of sperm in the semen in one patient.

On two different occasions, severe oligospermia documented by serial seminal fluid analyses was present coincident with methotrexate administration. Of approximately 339 patients evaluated at a private infertility service over a 5-year period, 24 couples underwent homologous artificial insemination (AIH). Nineteen of these were performed to circumvent the problem of oligospermia, and only one pregancy was achieved in this group; conceivably this pregnancy could have occurred by chance.
