Anesthetic Management for Separation of Omphalopagus Twins.
剣状突起結合体児分離手術の麻酔管理を経験した.症例は生後56日の女児で合計体重は6,480gであった.肝臓の一部を共有していたが他臓器に奇形はなく,交差循環もほとんど存在しなかった.麻酔導入はセボフルランでの緩徐導入とし,マスク換気が可能であることを確認した後,筋弛緩薬投与し気管挿管を行った.術中・術後は問題なく経過し,術後13日目に退院した.結合双体児分離手術の麻酔では合併奇形,重要臓器共有や交差循環の評価などの術前準備に加え,気道確保,術中出血への対応など,その管理は特殊で煩雑となる.スタッフ間で術前カンファレンスやリハーサルを行い,各自が病態の把握と術中の役割分担を確認しておくことが重要である.
