Ruptured Pancreaticoduodenal Artery Aneurysm
症例は59歳,男性.早朝より誘因のない腹部圧迫感,背部痛が出現したため来院した.膵炎,腹部手術および外傷の既往はなかった.腹部造影CTで膵頭部背面の造影される腫瘤を認め,血管造影により上前膵十二指腸動脈領域の動脈瘤破裂と診断した.血管内治療により塞栓術を試みたが病変部のみの塞栓は困難であり,腹腔動脈幹は描出されず,虚血による甚大な臓器障害が懸念されたため中止し,緊急手術を行った.Kocherの授動術を行い十二指腸外側より進入し,膵背面から出血点をコントロールした.そののち血腫を除去し,出血点を直視下に確認し病変部のみを縫合止血した.膵頭部,肝臓など周辺臓器の血流は十分に保たれていたため血行再建などの追加手術を行わなかった.術後経過は良好である.後腹膜血腫の診断には造影CTが有用であったが ...
