膵十二指腸動脈瘤破裂の1例を経験したので報告する.症例は55歳,女性.主訴は背部痛,臍周囲痛である.入院前日より背部痛を認め,その後突然臍周囲痛をきたし来院した.腹部US, CTでは,膵頭部領域に11×8×4 cmの内部不均一な血腫を認め,上腸間膜動脈,門脈が圧排されていた.腹部MRIでは, T1にて高信号, T2にて低信号を示し造影効果がないことから血腫と診断された.腹部血管造影では,下膵十二指腸動脈からの造影で前後下膵十二指腸動脈にそれぞれ1個の動脈瘤(8×11mm,3×11mm)を認めた.以上より膵十二指腸動脈瘤の破裂による後腹膜血腫の診断にて手術施行.術中所見では,血腫による癒着が高度であり,また下膵十二指腸動脈からの動脈瘤を検索したが動脈瘤を同定することができなかったため ...
