症例は78歳,女性.平成15年4月,膵頭部に3cm大の腫瘤像を認め分枝型IPMNと診断され,以後外来で経過観察されていた.平成18年5月,下腹部痛,微熱が出現し,当院に入院となった.CT上,腫瘍の増大傾向を認め,嚢胞内に新たに充実性病変を認めた.ERCP検査にて主膵管は数珠状に拡張し,嚢胞内に充実性成分と思われる欠損像を認めた.膵管内超音波検査にて嚢胞内に充実性の腫瘤を認め,生検ではIPMN with moderate dysplasiaであった.ERCP5日後,閉塞性黄疸を認めたためERBDチューブを留置した.減黄後,膵頭十二指腸切除術を施行した.術後の標本検索では,嚢胞と総胆管に直径約1cmの瘻孔を認めた.病理検査上,瘻孔部に腫瘍細胞は認めなかった ...

A CASE OF INTRADUCTAL PAPILLARY MUCINOUS ADENOCARCINOMA PENETRATED TO THE COMMON BILE DUCT CAUSING OBSTRUCTIVE JAUNDICE DUE TO MUCUS

症例は72歳男性.心窩部痛および黄疸を主訴に当院受診となった.腹部超音波検査および腹部CTにて,膵頭部に多房性の嚢胞性病変,著明な主膵管の拡張と軽度の胆管の拡張を認めた.また, PTCD造影で膵胆管瘻形成が疑われた.総胆管への穿破を伴う粘液高産生性膵管内乳頭粘液性腺癌と診断し,膵頭十二指腸切除術を施行した.病理学的には,微小浸潤を伴う乳頭腺癌が主膵管から分枝膵管まで進展しており,膵胆管瘻周囲には腫瘍細胞を認めなかった.本症例は粘液貯留により引き起こされた嚢胞壁の圧上昇に伴う自潰により瘻孔を形成し,粘液により閉塞性黄疸をきたした,と考えられた.
