Perilunate dislocations and fracture-dislocations: A multicenter study

A series of 166 perilunate dislocations and fracture-dislocations from 7 centers was retrospectively studied. The diagnosis was missed initially in 41 cases (25%). A classification system was presented. The perilunate fracture-dislocations were more frequent than the perilunate dislocations at a ratio of two to one.

Radial and Volar Perilunate Transscaphoid Fracture Dislocation

An unusual volar and radial perilunate transscaphoid fracture dislocation is reported. Treatment of perilunate transscaphoid fracture dislocations by closed reduction often produces a poor result. Early open reduction may be followed by a good result, but if open reduction is impossible, proximal row carpectomy is advised.

Transscaphoid Perilunate Fracture-Dislocation

Sixteen patients with 17 transscaphoid perilunate fracture-dislocations were treated by open reduction and pin fixation. Open reduction was performed through a volar approach in only one wrist, a dorsal approach in nine wrists, and combined volar and dorsal approaches in seven wrists. Primary bone grafting was performed in four patients.

Perilunate dislocations and fracture-dislocations

27 patients (28 cases), were treated for perilunate dislocations in 20 and fracture-dislocations in 8. Capitate displacement was dorsal in 27 cases and palmar in 1 case, and in 16, the scaphoid was fractured. There was no substantial delay of treatment. In 8 patients treated with closed reduction, follow-up averaged 6 (1-15) years.

Diagnosis of injury and clinical and radiographic findings depend on the pattern of injury. Greater-arc injuries are characterized by complete loss of contact between the lunate and head of the capitate and one or more fractures of bones surrounding the lunate.
