Summary A case of carcinoma in a large pharyngoesophageal diverticulum is described. Simple resection, followed by radiation therapy, was ineffective. Review of the literature shows that only in cases of noninvasive carcinoma in situ type of lesions were the results following surgery good.

Pharyngo-esophageal (Zenker's) diverticula are rarely associated with tracheal fistulas; however, the diagnosis should be suspected in the diverticulum patient who presents with spasms of coughing while eating since, if found, it changes the surgical management.

PHARYNGOESOPHAGEAL DIVERTICULUM: TECHNIQUE OF REPAIR

Pharyngoesophageal diverticulum is an acquired defect resulting from an incoordination of the cricopharyngeal muscle. Common symptoms are dysphagia, regurgitation, and aspiration. The defect is repaired through a cervical incision and should include a diverticulectomy and a myotomy. Results are excellent and complications are unusual.
