Optimizing surgery in hidradenitis suppurativa

open access: yesJournal of the European Academy of Dermatology and Venereology, EarlyView.
Surgery in hidradenitis suppurativa should be matched to disease extent. A structured six‐step approach—comprehensive assessment, optimal timing, ultrasound‐guided mapping, tailored intervention, individualized reconstruction and structured wound care—integrated with targeted systemic therapy optimizes outcomes and reduces recurrence.

Efficacy of Local Treatment in Patients With Rheumatic Diseases Affecting the Temporomandibular Joint—A Systematic Review

open access: yesJournal of Oral Rehabilitation, EarlyView.
This systematic review found limited evidence for local treatments of temporomandibular joint involvement in rheumatic inflammatory arthropathies (RIA). Based on 33 studies with moderate to high risk of bias, patients with juvenile idiopathic arthritis (JIA) showed better outcomes after total joint replacement than other RIA patients in terms of mouth ...

Inflammatory and Immunological Basis of Periodontal Diseases

open access: yesJournal of Periodontal Research, EarlyView.
The periodontal lesion emerges as an evolving immunological battlefield, where host–microbiome interactions, dysregulated immune responses, fragile resolution mechanisms, and inflammophilic dysbiosis converge to shift the balance from homeostasis to unrestrained tissue destruction.

Autoimmunity and Periodontitis

open access: yesJournal of Periodontal Research, EarlyView.
In a microbe‐driven inflammatory environment, peptidyl‐arginine deiminase (PAD) enzymes from neutrophils and Porphyromonas gingivalis citrullinate both microbial and self‐antigens. B cell presentation of citrullinated or self‐mimicking epitopes activates T cells that assist B cells in antibody isotype switching, affinity maturation, epitope spreading ...
