Reproducibility in circadian rhythm of ventricular premature complexes

To evaluate the existence and reproducibility of a circadian rhythm of ventricular premature complexes (VPCs), 38 patients (mean age 57 +/- 17 years) with greater than or equal to VPCs/hour were studied with 24-hour electrocardiogram Holter monitoring. Nineteen patients had coronary artery disease and 19 had structurally normal hearts.

Premature ventricular contractions (PVCs) are frequently encountered, and management is determined by symptoms, precipitating factors, and the presence of underlying cardiac disease. No treatment is indicated in patients with asymptomatic PVCs in absence of cardiac disease.

Frequent premature ventricular complexes (PVCs) can lead to a reversible cardiomyopathy. Frequent PVCs can also be a marker for the presence of structural heart disease. Both issues need to be addressed when a patient with frequent PVCs is evaluated. The PVC burden needs to be quantified and a predominant PVC morphology, if present, must be identified.
