Summary Endocarditis caused by Propionibacterium acnes is very rare. A new case is reported, involving a native mitral valve. Transesophageal echocardiography is essential for a rapid diagnosis. There are no guideliness on antibiotic therapy.

Propionibacterium acnes is a gram-positive pleomorphic rod-shaped anaerobic saprophyte of the skin, mouth and upper respiratory tract. Although associated with acne vulgaris, it is otherwise reported as a human pathogen only rarely, in various infections, notably cutaneous and osteoarticular, and in endocarditis.

The evolving role of Propionibacterium acnes in acne

Propionibacterium acnes is a member of the resident cutaneous flora. Sebaceous follicles involved in acne are characterized by the accumulation of abnormally desquamated corneocytes and excess sebum-the microcomedo. This environment provides ideal growth conditions for P acnes. Several orders of magnitude level of P acnes are found in microcomedos.
