卒中作为我国成人致死致残的首位病因，给社会和家庭造成了沉重的经济负担。既往以院内救治为主的医疗模式虽不断完善，但卒中发病率仍呈上升趋势。为此，我国正积极推进卒中防治策略转型，构建预防干预与临床救治并重的综合防控体系。通过强化全民健康教育网络、优化以社区健康服务中心为枢纽的高危人群筛查管理机制、创新社区卒中防治地图体系，形成未发病时、疾病早期和中后期三级预防闭环管理。这一体系已成为降低卒中发病率、致残率、死亡率及改善患者生存质量的关键路径。本文重点论述社区卒中防治地图体系的系统化构建策略 ...
