New England Journal of Medicine, 1993To the Editor: We report a case of muscle weakness induced by herbs that is associated with the boom in unconventional therapy. A 69-year-old man had an eight-month history of painless, nonfluctuating weakness, primarily of the lower extremities, without other associated neurologic or constitutional symptoms.

Phenytoin induced vitamin d deficiency presenting as proximal muscle weakness
A 6-year-old girl presented with proximal muscle weakness of lower limbs. She was receiving phenytoin for epilepsy for 2 years. Serum phenytoin level was within therapeutic range. Serum 25(OH) vitamin D was low (5 ng/mL) and serum parathyroid hormone level was high.

Five cases histories are reviewed here in which Duchenne muscular dystrophy had been mistakenly diagnosed 3 to 12 years previously and the correct diagnosis was made only after repeat muscle biopsy or biopsy review.

Seminars in Pediatric Neurology, 2008Proximal muscle weakness, often indicative of a myopathy, has a broad differential diagnosis in children. We present a case of an adolescent boy with proximal weakness and a mildly elevated creatine kinase. He was found to have spinal muscular atrophy type III rather than a myopathy.

Chronic Renal Failure Presenting as Proximal Muscle Weakness in a Child
Journal of Child Neurology, 1986A 13-year old boy presented with a three-year history of slowly progressive proximal muscle weakness, particularly involving the lower extremities. Chronic renal failure was uncovered in the course of his evaluation. Urologic investigation showed small and poorly functioning kidneys with a BUN of 118 mg/dL and a creatinine of 10.7 mg/dL.

[Immobilizing muscle weakness accentuated in leg and proximal muscles].
Praxis, 1998A 54 year old waiter was referred to the hospital because of proximal muscle weakness, most pronounced in his legs, which progressed to an inability to stand or walk within weeks. Myopathy was diagnosed based on the muscle biopsy findings and myositis was ruled out by laboratory and biopsy results.

[Myalgia and proximal muscle weakness in a young patient].
Praxis, 2009We report a case of severe vitamin D-deficiency in a 32 year old black woman living in middle Europe since 9 years suffering from chronic pain of the lower back, the neck and the proximal right thigh, as well as weakness mainly in the pelvic girdle. Vitamin D plays an important role in calcium homeostasis and muscle strength.
