Possible Paraneoplastic Amyotrophic Lateral Sclerosis Presenting as Respiratory Failure in a Breast Cancer Patient: A Case Report

In a breast cancer patient with acute hypercapnic respiratory failure, paraneoplastic ALS was diagnosed despite negative onconeural antibodies. Red flags included diaphragmatic paralysis, ESR of 120 mm/h, and CSF protein of 246 mg/dL. Clinicians should consider this diagnosis even when antibody panels are negative.

ABSTRACT A 19‐year‐old female with schizophrenia developed severe mixed tardive syndrome (choreoathetoid dyskinesia with prominent tardive dystonia) after antipsychotic treatment, which remained debilitating despite deutetrabenazine, benzodiazepines, baclofen, and amantadine.
