Pyloroplasty for Refractory Gastroparesis

Gastroparesis is a chronic gastric motility disorder in which the pathophysiology mimics a post-vagotomy state. Pyloroplasty is beginning to emerge as a successful drainage procedure for refractory gastroparesis. Here we report our experience using pyloroplasty in the surgical management of diabetic and nondiabetic gastroparesis.

Esophagogastrectomy without pyloroplasty

There is no consensus on the need for pyloroplasty after esophagectomy or proximal gastrectomy with an esophagogastrostomy and vagotomy. Arguments for routine pyloroplasty include prevention of postoperative delayed gastric emptying. Arguments against include prevention of postoperative dumping syndrome and bile reflux esophagitis.

Pylorojejunostomy as an Alternative to Pyloroplasty

Pylorojejunostomy has been successfully substituted for pyloroplasty when the latter is technically difficult. Heidenhain pouch studies in dogs indicate that acid secretory effects of pylorojejunostomy and pyloroplasty are comparable and similar to those of the control period and superior to those of gastroenterostomy.

A modification of the standard Heinecke-Mickulicz pyloroplasty is described. This is particularly useful in the treatment of large posterosuperior duodenal ulcers with haemorrhage (and perforation) and excludes the ulcer crater from the new gastroduodenal channel.
