Vitamin B6 (VB6) is a vital coenzyme for δ‐aminolevulinic acid synthase (ALAS) in heme biosynthesis. We report a 49‐year‐old male with severe microcytic anemia and ringed sideroblasts initially diagnosed as myelodysplastic syndrome (MDS).
