IT HAS been rather unfortunate that rhinoplasty was originated in obscurity and its progress hindered by the disapproval, even antipathy, of the great mass of the medical profession. Each rhinoplastic surgeon not only made his own errors but repeated the errors of others.

Nasal dynamics were studied on 87 patients undergoing rhinoplasty of one zone or two distant nasal zones. Statistical analysis of the result revealed that reduction of the nasion area, besides setting the soft tissue back, gave the appearance of increased intercanthal distance and lengthened the nose.

Rhinoplasty presents a unique set of challenges for the cosmetic surgeon. Complications may arise from inadequate diagnosis, errors in surgical technique, or variations in the patient's anatomy or healing response. Complications as a result of overly aggressive surgery may also have functional consequences and be harder to correct.

Secondary Rhinoplasty Following Open Rhinoplasty
Plastic and Reconstructive Surgery, 1995On the basis of 56 cases of secondary rhinoplasty following an initial open rhinoplasty, I conclude that secondary surgery is safe and effective. Either an open or closed approach can be used in most cases with little risk of skin necrosis or poor scar formation. The decision as to which approach to use depends on numerous factors. In both primary and secondary rhinoplasty, favorable illusion may be created by cartilage grafting of various nasal anatomic components, as well as rearranging components in relationship to one another. Recognizing which patients will benefit from the procedures described herein can lead the rhinoplasty surgeon to improved long-term outcomes.
