Pneumococcal Sacroiliitis

We report an unusual case of Streptococcus pneumoniae sacroiliitis in a previously healthy 31-year-old woman. Six cases of pneumococcal sacroiliitis have been reported; the only two cases in adults occurred in young women in the preantibiotic era. Our patient had fever and a depressed level of consciousness, with subsequent right buttock and thigh pain.

Pyogenic sacroiliitis

Seven definite and three probable cases of pyogenic sacroiliitis are presented and compared to 72 cases found in the English literature. Patients may present with a subacute localized or an acute systemic illness. Six of our patients were parenteral drug abusers.

Septic sacroiliitis

Two cases of septic sacroiliac (SI) joint arthritis are presented to illustrate the difficulty of diagnosing and treating this uncommon osteoarticular infection. The patients presented are a 68-year-old woman with septic sacroiliitis caused by Streptococcus agalactiae and a 20-year-old man with Salmonella infantis infection involving the SI joint.

[Septic sacroiliitis].

Five patients with septic sacroiliitis, 3 unilateral and 2 bilateral, are described. The illness was acute, with high fever, low back or buttock pain, severe sacroiliac tenderness and elevated erythrocyte sedimentation rate. Staphylococcus aureus was isolated from the blood in 3 cases and Salmonella typhi in 1.

Infantile infectious sacroiliitis

Two infants with pyogenic sacroiliitis are presented. In both babies, the clinical findings were initially attributed to septic arthritis of the ipsilateral hip. The correct diagnosis was not established in either infant until radionuclide scanning showed increased uptake in the ipsilateral sacroiliac joints.
