These lesions appeared 2 months after a cholecystectomy operation, which had been performed 4 years ago, and showed unilateral progression.

Her symptoms first appeared 2 years earlier in the mid region of her neck. After a few months, the lesion enlarged to the left side of her neck and became crusted.

Scrofuloderma (tuberculosis colliquativa cutis)
British Journal of Dermatology, 1996A 16-year-old girl presented with long-standing purulent sinuses to the skin overlying the lacrimal ducts, which had evolved into ulcers and resulted in hypertrophic scarring. The ulcers were serpiginous, with an uneven soft floor, and undermined edges. Histological examination revealed non-caseating tuberculoid granulomas in the dermis.

70歳,男性。当科初診の約4ヵ月前より頚部左側に発赤,腫脹が出現した。切開され,排膿を認めたが軽快せず,徐々に同部に潰瘍を形成した。難治であったため,当科を受診した。頚部左側に径4cmの痂皮を伴い,周囲やや発赤した潰瘍を認めた。皮膚生検では,Langhans型巨細胞がみられ,胸部X-Pにて左上肺野に結節性陰影,頚部MRIにて左鎖骨上窩に数個のリンパ節腫大を認めた。また潰瘍部の抗酸菌培養から結核菌が同定され,皮膚腺病と診断した。イソニアジド,リファンピシン,エタンブトール3剤併用療法にて約2ヵ月後に同部は上皮化した。近年高齢者の結核は増加傾向にあり,難治性潰瘍をみた場合,本症を念頭に置くことが重要である。
