Crisis in sickle cell trait

A 34-year-old black male with hemoglobin AS was admitted for renal failure, polydipsia, hypertension, schizophrenia, mental confusion, and visual hallucinations. Abnormal electrolytes were corrected by dialysis, but blood specimens were reported as hemolyzed with hyperkalemia.

HÆMATURIA AND SICKLE‐CELL TRAIT

SUMMARY Three patients are presented whose hæmaturia appeared to be a consequence of the sickle trait. One of these patients became very anæmic and showed radiological evidence of papillary necrosis. The natural history of this condition is reviewed and a regime for treatment is suggested.

The obstetrical experiences of 500 Negro women with sickle cell trait have been compared with those of 500 Negro women whose red blood cells did not sickle. There was no difference between the two groups of women in frequency of abortion, toxemia of pregnancy, prematurity, or perinatal death.

Sickle cell trait (SCT) can pose a grave risk for some athletes. In the past decade in NCAA Division I football, no deaths have occurred from the play or practice of the game, but 16 deaths have occurred from conditioning for the game, and 10 (63%) of these deaths are tied to SCT, an excess of up to 21-fold.
