A CASE OF COMPOSITE GOBLET CELL CARCINOID-ADENOCARCINOMA NEOPLASM OF THE SIGMOID COLON

症例は76歳, 男性. 腹部膨満感を主訴に当院を受診した. 腹部X線検査, 腹部CT検査にてS状結腸癌による大腸イレウスが疑われ, 人工肛門造設術を施行した. その後大腸内視鏡検査を行い, 病変部の病理組織検査にてカルチノイドが疑われS状結腸切除, D2郭清を施行した. 病理組織学的にS状結腸の病変は, 杯細胞カルチノイドと腺癌の混合腫瘍であった. 術後1年経過した現在, 再発なく生存中である. 杯細胞カルチノイドはそのほとんどが虫垂に発生し, 虫垂以外での報告は極めて稀である. また本症例は同一腫瘍内にカルチノイドと腺癌が合併していた. 杯細胞カルチノイドや腺癌の組織発生を考える上で興味深い症例であると考えられた. 若干の文献的考察を加えて報告する.

Perioperative and Oncological Outcomes of Colorectal Cancer Surgery in Obese Patients: A Multicenter Retrospective Study

The prognostic value of the log odds of positive lymph nodes (LODDS) across BMI categories was investigated in patients undergoing colorectal cancer resection. The exploratory evidence demonstrates that LODDS serves as a strong prognostic marker, and its adverse effect may be more pronounced in patients with higher BMI.

We report a LAMN surgical case whose appendiceal tumor penetrated the sigmoid colon wall. An 87‐year‐old man was referred for endoscopic resection (ER) of a colon polyp.
