Silent sinus syndrome.

We present a case of silent sinus syndrome (SSS) who underwent unilateral surgical endoscopic maxillary meatotomy. Orbital floor reconstruction is delayed after follow up. Enophthalmos recovered 8 months after the surgery and radiologic findings improved. SSS is a clinical entity that should be kept in mind in the differential diagnosis of enophthalmos

Silent sinus syndrome (SSS) is a rare condition, usually consisting of asymptomatic spontaneous collapse of the sinus walls and orbit floor. It is often found incidentally on MRI or CT imaging for other pathology work-up. The etiology of SSS remains controversial. Patients typically deny preexisting sinus disease or orbitofacial trauma.

Patients with silent sinus syndrome typically present for investigation of facial asymmetry. Unilateral, spontaneous enophthalmos and hypoglobus are the prominent findings at examination. Imaging of the orbit and sinuses characteristically show unilateral maxillary sinus opacification and collapse with inferior bowing of the orbital floor.
