An MRI‐based classification system integrating fibrosis extent (F1–F3) and inflammatory bone marrow edema activity (E0–E3) was developed for osteonecrosis of the femoral head (ONFH). Increasing fibrosis and edema severity were associated with higher collapse risk, worse pain, and poorer hip function.
