Stroke is one of the leading causes of death and disability worldwide. Although important advances in therapeutic approaches have been made, treatment is still far from satisfactory. Thus, major efforts should be made on stroke prevention. We present evidence‐based recommendations for primary stroke prevention.

Hypertension is the most important modifiable risk factor for stroke. There is clear and ample evidence of benefit for treating elderly hypertensive subjects up to the age of 80 with systolic pressures of over 160mmHg either alone or in combination with diastolic pressures over 90 mmHg.

More than 700,000 strokes occur annually in the United States--one every 40 to 50 seconds. Although stroke is one of the nation's most expensive diseases to treat, costing $41 billion per year, most strokes (perhaps as many as two thirds) are preventable.

It is now well recognized that stroke-prone individual scan be identified in the majority of instances and that effective preventive therapy is available. Cumulative risk factors enhance the potential for stroke. The challenge is to effectively intervene to substantially reduce stroke risk.

The best way to prevent a first-time stroke is to identify at-risk patients and control as many risk factors as possible. Some risk factors, such as smoking, can be eliminated; others, such as hypertension and carotid artery stenosis, can be controlled or treated to reduce the risk of stroke.

Over the past decade, statins have been proven to significantly decrease coronary events in primary and secondary prevention of coronary artery disease. Recent clinical trials have indicated that statins significantly reduce stroke risk in patients with vascular disease. Survivors of stroke and transient ischaemic attacks are at risk of a recurrent stroke, which is often more severe and disabling than the index event. Optimum secondary prevention of recurrent stroke needs rapid diagnosis and treatment and prompt identification of the underlying cardiovascular cause.

Secondary stroke prevention can reduce the risk of recurrent stroke by approximately 90%. To achieve such a reduction, early implementation of preventative measures and administration of therapy appropriate to the underlying cause of the presenting transient ischemic attack or stroke are crucial.

Stroke Risk Factors and Stroke Prevention
Seminars in Neurology, 1998Stroke is the third leading cause of death and the leading cause of chronic disability in the United States. In the past several decades, case series, case-control studies, and prospective cohort studies have successfully identified nonmodifiable risk markers for stroke, such as age, gender, race, ethnicity, heredity and several well-established ...
