Ultrasonic Ventriculostomy Stylet
Neurosurgery, 1995I describe my design for a ventriculostomy stylet fitted with a phase array ultrasonic imaging device in its tip. Theoretically, this would allow identification of the ventricles and their exact location at the bedside, without the need for expensive stereotactic equipment.

Failure to properly use the stylet when withdrawing a spinal puncture needle may cause aspiration of a lumbar nerve root and adjacent arachnoid, thereby fixing the nerve in the epidural space. Root fixation may cause a painful syndrome, requiring laminectomy and replacement of the nerve root within the subarachnoid space.

An optical stylet was developed which can be inserted into an endotracheal tube, and the passage through the vocal cords and position of the cuff in the trachea could be controlled visually. It is of great help in difficult intubations in adults and children.

When lighted stylet intubation is done correctly, the procedure can be very safe, with very little difference in outcome from that of primary laryngoscopy. (Note: several lighted stylet devices, such as the Trachlight TM and Light Wand TM , are no longer being manufactured, but these devices are still in use.)
