Subtotal thyroidectomy and total thyrodectomy.
甲状腺癌に対して甲状腺亜全摘・全摘術を施行する場合に, 再発のない様に手術をすることは言うまでもない。それ以外に留意すべき点としては, 反回神経と上喉頭神経外枝及び上皮小体の処理であると思われる。本稿では, 上記の留意点を中心として手術手技を手術の各場面に分けて述べた。粘膜剥離子を多用することにより, 出血が少なく, 神経を損傷する危険性の低い手術が可能である。また, 前頸筋は腫瘍の浸潤がなければ出来る限り保存している。反回神経の同定を頭側で行う方法について述べた。これは, 大きな腫瘍が下極に存在したり, 気管傍転移の多発する症例や, 又, 反回しない反回神経症例の場合に有用と思われる。

Postoperative parathyroid function was evaluated in 24 total thyroidectomy and 8 subtotal thyroidectomy patients seen by our department between January 1995 and July 1997. Parathyroid function was assessed by measuring the level of serum intact parathyroid hormon (intact-PTH).
