There is a problem in defining the occurrence of sudden unexplained death in persons with epilepsy (SUDEP). The diagnosis of SUDEP in the United States is under-used as many do not use the term on the death certificate. SUDEP is found to be more prevalent worldwide than assumed.

Epilepsy is a chronic condition that affects approximately 2.3 million people in the United States.1 Approximately one-third of people with epilepsy continue to have seizures despite adequate medication trials and adherence to medical recommendations.2 People of all races and backgrounds, young and old, are affected by epilepsy from a variety of ...

Rationale Fenfluramine substantially reduces rates of sudden unexplained death in epilepsy (SUDEP) in Dravet syndrome (Cross JH et al, AES 2020). Fenfluramine reduces respiratory arrest in the DBA/1 SUDEP mouse model (Tupal and Faingold, Epilepsia. 2019).

Case-control study of SUDEP [PDF]
To examine the influence of various factors on the risk of sudden unexpected death in epilepsy (SUDEP).The authors investigated 154 cases in which a postmortem examination was performed. Each case had four controls with epilepsy from the community, matched for age and geographic location.

Postictal Central Apnea as a Cause of SUDEP: Evidence From Near‐SUDEP Incident
Epilepsia, 2000Summary: While undergoing video‐EEG monitoring, a 20‐year‐old woman had a 56‐second convulsive seizure, after which she developed persistent apnea. The rhythm of the electrocardiogram complexes was unimpaired for approximately 10 seconds, after which it gradually and progressively slowed until it stopped 57 seconds later.

A 'going ape' model for SUDEP? [PDF]
Despite growing interest in both the clinical and scientific aspects of sudden unexpected death in epilepsy (SUDEP), knowledge about the disease mechanisms remains limited. The epileptic baboon—the first natural nonhuman primate model of SUDEP—offers a potential model to evaluate the genetic factors and pathophysiology underlying this mysterious ...

Pathology, 2014
Those dying suddenly on a background of a history of epilepsy (SUDEP) are seen frequently in the coronial setting. Issues around death certification will be discussed, along with the approach to the post-mortem examination, purported mechanisms of death in SUDEP and the emerging role of genetic studies in epilepsy related deaths.

A dietary supplement for SUDEP prevention?
Nature Reviews Neurology, 2016A new study has shown that dietary supplementation with a serotonin precursor reduces seizure-induced respiratory arrest (S-IRA) and death in mouse models. Development of strategies to prevent S-IRA is paramount, because S-IRA is thought to be associated with a high risk of sudden unexpected death in epilepsy (SUDEP).
