A CASE OF TALCOSIS

44才,男子.昭和39年5月より凾館市内滑石工場にて滑石粉砕工として従事.軽度のせき,体動時の息切れを訴え精査のため来院した.胸部X線写真では両側全肺野に網目状陰影がみられ,右上中および左上肺野に融合した塊状陰影がみられ,その一部に透亮像が認められた.肺機能検査では残気量,残気率の増加,肺拡散能力の低下と低酸素血症がみられた.生検肺の病理組織学的所見は,中等度の線維症像および,針状結晶を呑食した多数の呑食細胞がみられた.生検肺を灰化しX線回析像をもとめると,滑石がほとんどで少量の石英が検出され,本症例は滑石と石英が撰択的に肺に沈着したと考えられた.滑石粉末には滑石のほか種々の結晶が混在するので,滑石肺の病態を理解するためには詳細な鉱物学的検索が必要であることを述べた.
