Despite astounding progress in the biochemical management of Parkinson's disease in particular and of other movement disorders, there are still patients disabled by severe tremor and not by bradykinesia in whom thalamotomy remains the treatment of choice.

Tremor is a common movement disorder that can be disabling, and its initial treatment is in the form of medical therapies. Often patients are refractory and seek surgical intervention. Treatment options for these patients include surgical radiofrequency thalamotomy and deep brain stimulation. There are a subset of patients who, for various reasons, are

Since the 1960s, stereotactic surgery for tremor has targeted the ventral nuclear group of thalamus. According to Hassler’s classification (1), the nuclei in the ventral nuclear group, from anterior to posterior, are a pallidal relay nucleus (ventral oral, Vo), a cerebellar relay nucleus (ventral intermediate, Vim), and the principal somatosensory ...

Thalamotomy for Alleviation of Intractable Pain
Stereotactic and Functional Neurosurgery, 197417 patients had thalamotomies for alleviation of pain due to malignancy since January, 1970. In 12 of them, thalamic electrodes were implanted through occipital burr holes for subsequent radiofrequency thalamotomies. In 4 cases, the lesions were made by the bifrontal approach and 1 case was approached from the left occipital and right frontal.

Thalamotomy for Psychopathic Behavior
Southern Medical Journal, 1975Six patients characterized by psychopathic behavior were treated with lesions placed in the center median nucleus of the thalamus. There was marked improvement of the psychopathic behavior in five patients. One patient with unilateral thalamotomy was parqially improved. Four of the six patients have been productive and earn a living. The other two have

Bilateral Focused Ultrasound Thalamotomy for Essential Tremor (BEST‐FUS Phase 2 Trial)
Movement Disorders, 2021In patients with medically refractory essential tremor, unilateral magnetic resonance–guided focused ultrasound thalamotomy can improve contralateral tremor. However, this procedure does not address ipsilateral symptoms.

Revisiting bilateral thalamotomy for tremor
Clinical Neurology and Neurosurgery, 2017MRI guided focused ultrasound surgery (MRgFUS) has been FDA approved for unilateral treatment of essential tremor (ET). Before this non-incisional lesioning method can be applied to the treatment of both hemispheres the previous experience with bilateral thalamic ablation must be addressed.

Probabilistic Refinement of Focused Ultrasound Thalamotomy Targeting for Parkinson's Disease Tremor
Movement DisordersThere remains high variability in clinical outcomes when the same magnetic resonance image‐guided focused ultrasound (MRgFUS) thalamotomy target is used for both essential tremor (ET) and tremor‐dominant Parkinson's disease (TDPD).
