The First Successful Separation of Conjoined Twins – Thoracoomphalopagus in RSUP Prof R. D. Kandou Manado: A Case Report

Conjoined twins, a rare congenital anomaly, occur in approximately 1 out of every 100,000 births, and not all cases result in survival. Only about 25% of conjoined twins who are born alive survive and then decide whether they can be separated or not. This separation of conjoined twins is the first case carried out in our institution.

A Rare Case of Deradelphus Cephalo-Thoracoomphalopagus in Lamb

Male cross-breed lamb conjoined twins were sent for examination. The twins came from a farm located in the outskirts of Palermo. The twins were spontaneously delivered and found dead by the farmer. X-ray examination and necropsy were performed. The deformity was identified as deradelphus cephalo-thoracoomphalopagus.

Complete conjoined twins (duplicitas completa) are conjoined twins whose spinas are totally separated. Approximately half are stillborn, and a smaller fraction of pairs born alive have abnormalities incompatible with life. The case under report was a thoracoomphalopagus conjoined twins which had been delivered by caesarean operation on a Simmental cow.

Preoperative evaluation of intrathoracic and intraabdominal organs is very important before surgical separation of thoracoomphalopagus conjoined twins. Hepatic fusion is one of the major obstacles to surgical separation. We evaluated such a case and found noninvasive scintigraphic procedures to be of value.

Twins may be dizygotic or monozygotic. Of the monozygotic twins, 25% to 30% are dichorionic and 70% to 75% are monochorionic. Whilst all the dichorionic twins are diamniotic, very occasionally some monochlorionic ones may be monoamniotic; some of these may be conjoined. A case of conjoined twins was diagnosed during pregnancy.

Twenty-two cases of conjoined twins have been collected in Thailand. The numbers occurring in the various parts of Thailand are listed. No known cases have occurred between the years 1968-1973. The extent of union between the conjoined twins is described and illustrated.

Ruptured omphalocele in thoracoomphalopagus conjoined twins.

A ruptured omphalocele is a very rare complication in conjoined twins. Some cases have prompted emergency neonatal separation of the twins with varying results. We report a case of ruptured omphalocele in a thoraco-omphalopagus twin managed by primary repair of the defect at the University of Nigeria Teaching Hospital, Enugu.
