ABSTRACT Paravalvular leak (PVL) is a recognized complication after surgical mitral valve replacement and may cause heart failure, pulmonary hypertension, or hemolysis when moderate or severe. Although redo surgery is the definitive treatment, it carries substantial risk in elderly patients or those with multiple prior cardiac surgeries.

Thoracoscopic epicardial AF ablation in heart failure improved LV systolic function irrespective of left atrial appendage occlusion (LAAO). Concomitant surgical LAAO was associated with fewer LPV reconnections without worsening postoperative diastolic function, while atrial tachyarrhythmia recurrence was similar between groups.

ABSTRACT Left atrial (LA) mass is most commonly presumed to be a cardiac myxoma, as it is approximately 75% of primary cardiac tumors, with a strong predilection for the LA. In rare instances, primary lung carcinoma may extend to the LA, creating an intracardiac mass indistinguishable from a myxoma on initial echocardiographic assessment.
