We present the case of a 62 year old patient admitted with acute chest pain. Coronary CT angiography demonstrated presence of a high grade lesion with pronounced remodeling and the impression of an intracoronary thrombus in the proximal left circumflex coronary artery.

The patient was on adequate warfarin treatment. At physical examination, she had a cold right leg without pulsations and a discoloured and patched skin.
