This 10‐year retrospective case series evaluated mid‐term pulmonary outcomes in children who underwent diaphragmatic plication for phrenic nerve palsy following cardiac surgery. The results reveal a restrictive ventilatory pattern and consistent post‐exercise oxygen desaturation, despite preserved functional walking capacity.

This case highlights the diagnostic challenge of mediastinal IgG4‐RD and the importance of considering it in the differential diagnosis of thymic tumours, even when serum IgG4 is normal.
