Trabeculectomies

Abstract This study comprises 277 primary open‐angle glaucomas in 239 patients followed up to 10 years after trabeculectomy. Sixty‐four per cent had capsular glaucoma and 36% simple glaucoma. The number of cases showing no progression of glaucoma damage and not needing additional medical therapy decreased slowly with time.

The more important surgical steps in a trabeculectomy are the creation of a lamellar-scleral flap, the excision of a block of limbal tissue without damaging underlying structures, the creation of a basal peripheral iridectomy, and, finally, the closing of the scleral flap to maintain fluid within the anterior chamber.

Trabeculectomy at the inferior limbus

Abstract. Fourteen eyes (12 with open‐angle glaucoma and 2 with secondary glaucoma) of 14 patients were operated by trabeculectomy at the inferior limbus. The surgical technique is described and results of 6–12 months follow‐up are reported. Complication rate was reasonably low.

In a critical review of trabeculectomy, some factors and steps that I consider important for its successful outcome are discussed. Trabeculectomy is a misnomer, because the success of this procedure does not depend on inclusion of trabecular meshwork or Schlemm's canal in the excised tissue.

Angiogenesis: Trabeculectomy and Bevacizumab

The use of bevacizumab for the treatment of proliferative diabetic retinopathy, neovascular glaucoma, neovascular age related macular degeneration, and cystoid macular edema has been extensively reported upon. From the published case reports available, bevacizumab may be a useful adjunct to the trabeculectomy procedure.

A Histologic Survey of Trabeculectomy

Thirty-two tissue blocks removed during trabeculectomy were examined histologically, 16 by both scanning electron microscopy and light microscopy, and 16 by light microscopy alone. Trabecular meshwork was identified in 27 specimens (84.3%) and Schlemm's canal in 17 (53%), each found with equal frequency by either technique.
