Tranexamic acid is lysine-like drug that prevents the dissolution of the fibrin clot. It is associated with a significant reduction of bleeding, need of transfusion and mortality in several surgeries, such as cardiac or major orthopedic surgery.

Tranexamic acid is the trans-isomer of 4-aminomethylcyclohexane carboxylic acid. The initial investigations were made using a preparation containing a mixture of isomers, but it was subsequently found (Melander et al. 1964; Okamoto et al., 1964) that only the trans-isomer is antifibrinolytically active.
