Diagnosis of Lymphangitic Carcinomatosis by Transbronchial Lung Biopsy

The records of patients who had transbronchial lung biopsy at Walter Reed Army Medical Center between January 1974 and September 1977 were retrospectively reviewed. Nine patients were found whose clinical and radiographic presentations were compatible with lymphangitic carcinomatosis.

In Sweden-like most of the Scandinavian countries — sarcoidosis is an important medical problem. The prevalence is high, usually more than 50 cases per 100.000 of the adult population. In the Northern parts of Sweden more than 100 sarcoid patients per 100.000 of people have been found on mass chest radiography.

The clinical diagnosis of lymphangitic carcinomatosis of the lung has generally been confirmed by open lung or transthoracic needle biopsy, precedures with significant morbidity and mortality, particularly in patients with respiratory insufficiency. We present six cases of lymphangitic carcinomatosis diagnosed by transbronchial biopsy.

[Experience with transbronchial lung biopsy].

The histological results of transbronchial lung biopsies have been analysed in 109 patients with diffuse or localised lung diseases. This diagnostic procedure is relatively safe, well tolerable and can be carried out in outpatients. In diffuse lung diseases its use can replace the open lung biopsy.

In 40 patients with the ultimate diagnosis of sarcoidosis transbronchial lung biopsy during fiberoptic bronchoscopy revealed in 83% noncaseating granulomas.
