National Practice Patterns of VA ECMO and Left Ventricular Mechanical Unloading for Cardiogenic Shock

open access: yesArtificial Organs, EarlyView.
Consensus surrounding the management of VA ECMO and left ventricular mechanical unloading for cardiogenic shock (CS) is limited. We thus conducted a national survey of practice patterns in the treatment of CS to characterize such variability, finding large heterogeneity in CS diagnosis, VA ECMO initiation, and LVMU strategies.

When Machine Perfusion Falls Short: Identifying Risks of Liver Graft Failure After Normothermic Machine Perfusion

open access: yesArtificial Organs, EarlyView.
Analysis of 4928 normothermic machine perfusion liver transplants reveals that recipient factors, particularly MELD ≥ 35, surgical complexity, ICU status, and functional impairment are stronger predictors of graft failure than donor characteristics, emphasizing the continued importance of clinical judgment in patient selection.

An Optimized Multi‐Stage Drainage Cannula Design for Venoarterial Extracorporeal Membrane Oxygenation

open access: yesArtificial Organs, EarlyView.
This study utilized an extensive optimization involving n = 540 design of experiments to produce a superior drainage cannula design. Compared to the clinically used Maquet model, our optimized design demonstrated reduced blood stagnation volumes, increased cannula tip velocities, and higher upper body drainage, potentially minimizing risk of thrombosis

Geometry‐Related Technical Complications After LVAD Implantation: Impact on Device Dysfunction and Clinical Outcomes

open access: yesArtificial Organs, EarlyView.
Preventable geometric failure modes, such as outflow graft kinking and inflow malposition, frequently cause HeartMate 3 dysfunction. This study identifies these complications as key drivers of device‐related morbidity, highlighting the importance of optimized surgical positioning to ensure long‐term device performance and improved clinical outcomes ...
