A CASE OF ZOLLINGER-ELLISON SYNDROME
膵腫瘍に高度な腎酸過多と消化性潰瘍が合併する状態は一つのclinical entityとして, Zollinger-Ellison症候群と呼ばれ,本邦における報告例は少ないが,この症候群に留意して検索をおこなえばかなり発見できるのではないかと思われる.症例は35才の男.数年前より下痢,口渇,胃部不快感があり腹部腫瘤の疑いで試験開腹を受け膵頭部ラ島腺腫を確認された.多飲,多尿とともに倦怠感強く脱水状態で,胃,十二指腸に潰瘍があり,胃液は高酸を示した.水様性下痢と低K血症を伴ない,尿糖は陰性であつたが血糖曲線は糖尿病型を示した.尿崩症々状は高Ca血症と相まつてelectrolytes in balanceにもとづく腎障害のためと思われる.なお,上皮小体の機能亢進が考えられたが ...
