症例は47歳，男性。2，3日前から右下腹部痛が続き，その後に嘔気，悪寒および冷汗が出現したため救急外来を受診した。受診時は右下腹部に限局した圧痛および反跳痛を認めたが，発熱は認めなかった。腹部造影CT検査で上行結腸近傍に脂肪織混濁を認め，上行結腸垂炎と診断した。抗生剤治療を開始し，外来通院加療で軽快した。原発性腹膜垂炎は多くが自然軽快する予後良好な疾患である。臨床経過や腹部所見より鑑別診断として急性虫垂炎や憩室炎があげられるが，原発性腹膜垂炎は特徴的な画像所見を示すことが多いため ...
