The recognition of extrapituitary ACTH-secreting tumors is of great importance, not only theoretically but also in practice, in that it permits rational and radical therapy of the relative Cushing's syndrome and avoids more traumatic and risky surgery such as the destruction of the hypophysis and total adrenalectomy.

Endogenous Cushing's syndrome is a clinical state resulting from prolonged, inappropriate exposure to excessive endogenous secretion of cortisol and hence excess circulating free cortisol, characterized by loss of the normal feedback mechanisms of the hypothalamo-pituitary-adrenal axis and the normal circadian rhythm of cortisol secretion [2].
