In persistent AF, adjunctive dominant frequency‐guided ablation after pulmonary vein isolation did not significantly improve outcomes. Limited overlap between high‐frequency sites and low‐voltage areas suggests functional–structural dissociation, while right atrial low‐voltage burden predicts recurrence.
