A 39-year-old Ugandan student is described presenting with general malaise, fever and a pulmonary infiltrate. Open lung biopsy showing infarction and positive ANCA lead to a diagnosis of Wegener's granulomatosis and a treatment with immunosuppressive drugs was instituted.

Anti-neutrophil cytosolic antibodies (ANCA) and anti-endothelial cell antibodies (AECA) have been identified in a wide variety of disorders, but their pathophysiological role remains unclear. As ANCA are occasionally noted in patients with infectious disorders independently of any vasculitis process, we examined serum from patients with acute infection (n = 22) and septic shock (n = 57). Only two patients with acute infection were ANCA positive as determined by indirect immunofluorescence and western blot analysis.

There are few reports of IgA nephropathy (IgAN) with antineutrophil cytoplasmic antibody (ANCA)-associated glomerulonephritis.The authors report the clinical and pathological findings in 14 patients with IgAN and ANCA seropositivity.These retrospective cases consisted of 4 men and 10 women with a mean age of 44.4 ± 12.7 years.
